MedScore360 ("MedScore360," "we," "us," or "our") respects your privacy. This policy explains what information we collect through our website, medscore360.com (the "Site"), how we use it, and the choices you have.

This policy covers the Site only. Medical, claims, and treatment information handled through our Medical Provider Network (MPN) operations is governed by applicable California workers' compensation law and separate confidentiality obligations — not by this website policy. If you are an injured worker with questions about your medical information, contact your physician or your claims administrator for support. Do not share confidential medical information on any form on this site.

Information we collect

Information you provide. When you fill out a form on the Site — such as a contact, consultation, or physician inquiry form — we collect the information you submit, which may include your name, email address, phone number, company or practice name, and the content of your message.

Information collected automatically. When you visit the Site, we and our service providers automatically collect certain information through cookies and similar technologies, including your IP address, browser type, device information, pages viewed, time spent on pages, and the referring website. Our Site is hosted on HubSpot, which uses cookies for analytics and site functionality.

How we use your information

We use the information we collect to:
